SENIOR HIGH SCHOOL EARLY REGISTRATION
(For School Year 2019-2020)

Requirements for INCOMING GRADE 11:
- Photocopy of Grade 10 Report Card (with at least 2nd Grading Period)
- Photocopy of Grade 10 School ID
- Photocopy of PSA Certificate of Live Birth
- Fully accomplished BPC SHS Application Form (Avail it from Window 2)

Requirements for GRADE 12 TRANSFEREES:
- Photocopy of Grade 11 Form 138 (Report Card)
- Photocopy of Good Moral Certificate
- Photocopy of Grade 11 School ID

REQUIREMENTS FOR ENTRANCE EXAM
(For School Year 2019-2020)

INCOMING FRESHMEN
- Photocopy of Grade 12 Report Card (with at least 2nd Grading Period)
- Photocopy of PSA Certificate of Live Birth
- Barangay Clearance
- 1pc. 1x1 colored picrure
- Accomplished Application Form (from BPC)
- P50.00 Entrance Examination Fee Receipt
